It's all to do with respect. Showing your shoe soles is a sign of great disrespect in the Middle East, so throwing shoes is a mix of publicly displaying your lack of respect for Bush as well as being a violent action. If he wanted to cause damage and he didn't mind about showing that he didn't respect Bush he probably would have gone for a more heavy object to throw at him.

Guy's probably being received as a hero over there for what he did. It's a weird reflection of the culture, what with being a sign of great disrespect and how the man chose such a violent way to show his disapproval for Bush.

The funny thing is, if you watch the footage, the guy throws the first shoe, there's a slight pause by EVERYBODY as he takes off his second shoe, and than he throws the second one.
