Underrated in their own universe or underrated comics or underrated in their abilities?
You can have a superhero/villain who is underrated in their own universe but has a big fan following.
You can have a superhero/villain who is from unpopular or uncommon comics so while the hero/villain isn't necessarily underrated, he/she's relatively unknown.
You can have a superhero/villain whose true abilities are frequently overlooked.
Blade the vampire slayer, he's black, and a complete badass but no one even knows who he is nowadays.

The Punisher has gained a pretty significant cult following, so I won't try to pretend that he is too underrated, but I am a bit saddened by the fact that the franchise has never had a very well made movie, they all have some problems that are too big for a main stream audience to get past.
Though to be fair, the idea of someone as violent and ruthless as The Punisher may not appeal to the main stream audience to begin with
