Nowadays, the only thing that comes to mind is the zombie thing from the episode of Courage the Cowardly Dog with the stone slab and the different curses.
That damn thing STILL creeps me out, even to this day, so there's no fucking way in hell I'm looking up a picture.

I don't know if this counts as a monster, but the animatronics from Chuck E Cheese used to really scare me.

To lazy to check if anyone's said it already in this thread but Cell's first form was terrifying for me when i was a kid, i remember i used to be afraid to shower and go around my house alone whenever i saw an episode that him in that form.
The sewer monster from the X-Files gave me the fucking shits for years.
