I always thought the plastic surgeries were the most fucked up. They almost completely remove the face sometimes, and it's pretty potent. Still, I haven't seen anything to top Terror Tape so far. And yes, Terror Tape is SERIOUSLY messed up. At this point, I think it's too fucked up to actually be real. It's kind of like Blair Witch like that.
