I usually go for the smaller ones. Like somewhere within the range of 2 to 10 in photoshop terms.

Much better, but I think you should shift the entire hat a smidgen to the right.
Like, draw the doctor's head without the hat. Now, put the hat on top of it. The position of the hat is really awkward according to where his skull should be.

Yet the cobble stones don't seem to match the perspective, the angle of the seems is to straight opposed to the background. Do you see what I mean?
The distort transformation tool in PS or GIMP might save some time.
