I got the medal while collecting the last weapon.
I don't have every Skill, but I do have every Equipment, so it is possible that you could also need that or it just bugged for you.

The last weapon for me was the craftable Shadow Edge, which may have bugged it. I was talking about the "Upgrades" in the info, which I assume are health/energy upgrades.

Since the author is not capable of giving a set of rules in his game, maybe we can do so? I think, there are some useful tips for this game in the comments, but they don't cover everything, I believe. Also, I don't get it from lvl 14 onwards, since the major rule of blocking lighted tiles doesn't always apply here.

Huh, with spamming Tab and Space you can get pretty far in this game, but at the same time softlocking is possible. I cannot see a way to play this with the mouse.
