Origins of Venom.
Close, but not quite. Several of my searches for a pic of Spidey have ended in me reading about the history of Venom and his "children": Carnage, Riot, Lasher, Phage/Rampage, Agony, and Scream. Parts of Riot, Lasher, Phage, and Agony also joined together to form Hybrid. Carnage also has a son named Toxin.
In the mid-80s, Marvel comics released a 12 issue series called Secret Wars. The basic premise is that the Superheros and Supervillians for several Marvel series, as well as some new characters, were brought to a planet called Battleworld by a god-like character named Beyonder, where they were then forced to fight each other. The planet is filled with many alien weapons and artifacts.
During the battles, some of the hero's costumes get rather tattered (for shame!). Thor (Journey into Mystery/The Mighty Thor/The Avengers) finds some type of alien sewing machine to fix his, and Spidey goes out looking for the same. Instead he finds a canister with some black goo in it. When he touches the goo, it engulfs his body, and forms a costume similar to Spider-Woman's (whom Spidey had a crush on). Close enough. The costume augmented his strength and had unlimited webbing, plus could mimic the appearance of others, so he kept it when he returned to Earth.
As for why the goo was on Battleworld in the first place, it was a member of a nameless race of parasites that fed on adrenaline and other hormones while killing their hosts. This particular one was an outcast, wanting instead a symbiotic relationship, and was condemned to die via starvation. Enter Spidey.
You've got the rest pretty much right. Spidey (actually Reed Richards (Mr. Fantastic)) eventually discovers the suit is alive, is forming its symbiotic bond, and will eventually take complete controll of his body. They also discover that it is week against high frequency sonic waves. They remove it, but it escapes and tries to re-attach itself. Spidey goes to a church, where the bells are able to remove it again. The rejection mentally damages the symbiote, and causes it to begin hating Spidey. Coincidentally, fallen reporter Eddie Brock, who blames Spidey for the loss of his job and wife, is at the same Church praying for forgiveness for the suicide he is about to commit. The symbiote psychically detects his emotions, and takes him on as its new host. Their combined hatred for Spidey forms Venom.
Carnage comes much later. At a time when Brock and the symbiote are separated, Brock is put in jail. His cell-mate is serial killer Cletus Kasady. When the symbiote comes to rejoin with him, it is "pregnant." As Venom breaks out of jail, it leaves behind a seed that bonds with Kasady, forming the very much insane Carnage.
The seeds for Venom's other 5 "children" were forcibly extracted by a group known as the L.I.F.E. Foundation with the intent to form super-cops for a post-M.A.D. world. They all went crazy, with Scream eventually killing the other 4, remnants of which formed Hybrid.
Toxin is Carnage's "child." It is the 1000th in its generational line, which apparently causes genetic defects. Though more powerful than his father and grandfather combined, he is the mental equivalent of a 4 or 5 year old child. His host is NYC cop Patrick Mulligan, and he is the first fully "good" symbiote, though Hybrid also tends more towards superhero than supervillian.
