I can only think of 6 hard medals to get. (Not impossible but pretty hard):
1. Beat the Mage Master (Larry and the GNOMES)
2. Beat game on hard (Dad Game)
3. Beat the game with every character (Portal Defenders)
4. Buy a nuke (Manhaten Project)
5. Reach 50,000 miles (Toss the Turtle)
6. Defeat funky chicken (Can't remember the name...that one 2-d pico game wwhere you rescue your friends)

I was there, BUT there are ways to cheat.
K: Keep an object on the K key, and come back when it's time to remove it.
The Pianist: Copy/Paste from the guide.
Show Contestant: Tab key.
War Hero: Leave the computer on (foward/back goes faster)
Human Test Subject: Foward/back as many times as you can so it goes faster. Copy/paste the code. This is still one of the hardest medals out there to earn.
John Locke: 4 8 15 16 23 42.
I've tried forever to get 'Easy Way Out' for Enough Plumbers. That medal was a bitch to get.

Those were hard, but there are indeed harder... Look in Alkie Kong, OCD+, I wanna be the flash game, Ching-Chong Beautiful, Stickya Adventurya, Bomb Diggity, and the DREADED Lastman. (a game never to be mentioned in conversation.)
