Because most birth defects are caused by recessive genes. That is a gene that needs to be present in both matching chromosomes in order to be expressed. In other words BOTH parents must carry the gene in order for it to be expressed in offsrping.
If the two parents are related, they share a percentage of their genes (as they in turn inherited them from their parents). Thus the likelihood of a recessive gene that causes a defect being expressed is much higher in related couples than in unrelated couples.
It should be noted that related couples don't ALWAYS produce babies with defects (its possible that the parents don't carry any recessive defect genes, or that only of them inherited it from their parents and therefore only one can pass it on), just that the probability is significantly higher.
