Yep you can really get some fast gain depending what games (and/or movies with medals and/or other stuff with medals) you're doing.
If you went through all of selfdefiant's stuff in a week or if you went through all of Munguia's art submissions in a week, you'd rack up tons of easy points fast, for example. Ditto with medals that award for doing easy stuff like watching a video or clicking something in a video, going through a visual novel, etc...
Another thing to note is you can try to see what names seem to be popping up again and again in the games with medals, and try to clear out a bunch of their submissions. selfdefiant and Munguia would be two which have huge points totals in their submissions, but there are others.
And finally, while the max points on most submissions is 500 (with many having less than that), do make note of submissions and authors whose submissions are worth more than that. I assume this is some special permission granted by Tom Fulp himself to his favorite submitters or something.
Also it should be noted the difference between quantity and quality. While you can certainly boost your score huge amounts by doing these things, it's not the same as quality. There are many easy games with lots of medals or high-point medals. Conversely there are hard games with low point medals. Even if you just take average-ish medal point submissions, still it's significantly easier to earn, say, all the medals from 20 easy games than it is to earn them all from 1 hard game, etc...
So as such the medals toplists themselves aren't really indicative of player skill by themselves, as you'd really need to drill down into it to evaluate per person. A good approximation is a much higher score but much lower total games played tends to indicate a higher-skill player, but this isn't 100% either. It is, however, a good rule of thumb, as it tends to indicate players who have a high percent of finishing out games they've played rather than just playing a *lot* of games (but only obtaining the easy medals from them). Still possible they could have only played easy games though, so if you really want the scoop you're going to have to drill down into their medals pages and have a look through it all.

I didn't know about selfdefiant. Indeed, I played Munguia. And kind of as some good information, I also caught up on fortunacus, Artur-Felipe and Wiesi. Top it with a bit of Gamezhero.

Personally, I used to have a good ratio. And for example, I beat Icy Cave of Frustration. :)

fortunacus is one who has several games with greater-than-500 points. So indeed you could get large scores there if you got through his stuff. However, these games tend to be very hard as well, so this is less grind and more skill.
matt-likes-swords also has several games with greater-than-500 points medals (indeed many with 1000 points or more) but again also these games tend to be very hard, although certainly if you just went through and got only the easy medals from them, you could still get a lot.
Gamezhero is pretty mixed bag. Artur-Felipe I'm not familiar with.
Wiesi has some quite hard games, but it's probably easy enough to just get a lot of medals (but not all) from some of his stuff. I still have no idea how to get the "destroy x amount of some kind of boats" in several of his shark games, because even if you destroy literally every single one, not enough appear in the entire game, so I'm thinking there must be a trick to it or something.
Yep if you just went with the easily obtained medals you could get a lot. If you tried to fully complete out the games from those authors, gaining all medals, and succeeded, mad respect to you.
