For the perfectionist medal, your have to get every donut before entering the final level. And then collect the 13 donuts in the final level, it should be given during the final cutscene. If you already beated the game without doing this, you will need to reset your save file and start from the beggining.
I LOVE DONUTS!!! So I know this is a great game for me!
