There is an achievements button on the interface that will help you keep track of your medals.

The medals are not secret, I just found an issue with the NG system that they haven't fixed yet. You will still get credit for the medals on your userpages.

Getting All The Medals

1.Thumb Up Ur But: Don't Do Anything
2.The Gambler: Click the Use button, then click on the pool
3.Headshot!: Hit the harpoon or grab the harpoon then hit it
4.I'm Not dead Yet: Get the quarter, click on the quarter then click the machine ride, finally press the red button
5.Following Instructions: Hit the wheelbarrow but make sure the bag of cement isn't open. It's not open if there isn't any cement mix coming out of the bag. Get the bag, click on it, then click on the pool
6.Dive Right in: Click the Get button, then click the pool
7.Get Over Here: Get the harpoon and use it on the fat boy
8.Statueque: It's like number 5 but this time hit the cement bag then use it on the pool.
9.Butterfly Effect:It's like number 4 but instead hit the green button
10. I need A Bigger Boat: Get the harpoon then LOOK at the pool, the shark will come out and use the harpoon on the shark.
