Tremendous fun!

Positives:
+Great level design
+I loved the teleportation mechanism, it was very cleverly implemented
+Good music
+Mute button
+Medal times provided for extra challenge, but not enforced
+The limited respawn points + setting respawn points worked well.

Negatives:
-No separate mute options for music and sound effects
-Occasionally somewhat laggy
-No real way to pause aside from unfocusing the window
-Ending was a bit unsatisfying - a bit more fanfare might have been nice.

All in all, excellent work!
