Lavender Town Music.
I actually used to have the original Pokemon Green (in Japanese) on my GBA when I was kid, and remember playing through lavender town, and it used to creep me out so much, and it still does.
Imagine playing the original music at 4 am after you've just watched a horror movie, or something. x_x

The Haunted Mansion theme from Timesplitters Future Perfect. The design of the mansion plus that music creeped me out.
Honestly one of the most chilling themes I have heard in a video game. The setting of the current level didn't help either. Nor did the fucking spider. There's been a few from other games that give me the creeps, like Left 4 Dead, Beyond Good & Evil, TimeSplitters, even Ace Combat, but Rayman 2 comes out on top for me.
The playrooms from shadowman. Its so demented and what made it worst its that the playrooms area is a labyrinth.

When I was younger I would keep my Gameboy Color's sound off playing Pokemon due to Lavender Town's music being to creepy for my child self, and me believing that it would make my batteries last longer.

Giygas theme is a bit unsettling. I love it though, because it adds a lot to the atmosphere of the battle and helps to make it one of the most memorable final boss fights ever.
Also, I always thought Majora's Mask's final battle theme sounded a little creepy.
resident evil 2...or all of the older ones really, really everywhere you went it had some creepy music and then you hear a lound banging noise....that used to scare the shit out of me

All the music from the games that you guys are talking about is nothing compared to this. LSD had the most disturbing soundtrack in video game history IMO.
