It's Dan Kuroto a.k.a. Kamen Rider Genm from Kamen Rider Ex-Aid.

Kuroto is the head of the video game company Genm Corporation which is responsible for the Game Drivers our heroes use to cure their patients of the Bugster virus (have I told you that Ex-Aid is a doctor/gamer themed Kamen Rider season?).
But it's shown that he himself is working with the Bugsters and is the main villain who takes on the identity of Kamen Rider Genm, a darker coloured version of the protagonist, and later upgrades himself with Dangerous Zombie, a zombie game.

He actually first appeared in Kamen Rider Ghost during the final battle where he came and showed up Specter and Necrom using Shakariki Sports, a BMX bike game to summon a bike and defeated a huge group of Ganma foot soldiers that the two ghostly themed Riders had trouble with.
Not sure if it counts as a spoiler, but the form furthest to the right is Another OOO, a form he takes on in Zi-O (the anniversary season that's still going as of now) thanks to the villains of that series, the Time Jackers.

He is quite a popular villain, so I guess it made sense to bring him back. And there was also the fact that he was only available during these weeks and not during the Ex-Aid episodes.
He's also known for giving himself new titles and rewriting his name with different letters to reflect that, and it's pretty over the top.
