Pure awesomeness
Good creativity, the colors seems to adapt to the game nicely. On of the most presenting points is the big red eyeball in the middle of the head which demonstrates nice color and display.

Like Serephix said, the perspective really changes the thin, agile sister that we see in the game, to the unusual hulking mutated form in the picture. It actually took me a minute of looking to figure out why she looked so diffrent :P . Overall, very good attention to detail.
To clarify
I believe the right arm and left leg are both smaller because it's perspective. Part of her is closer to the "camera" Then the rest, making those parts bigger.

Truly great job dude, my only problem with this is that the right arm seems a bit too small. Other than that, everything else is amazing. You really captured the essence of Rapture and the Big Sister. Kudos to you :)
