Pretty fun but I got bored of it pretty quickly.

Translation: The game is fun and hard, the music is great and it has good graphics.
Really hard and random. And some items are much better than others, but it's generally very hard to know what an item will do without just trying it (if you've not obtained that item before). Some items even hurt you.
Good items to be on the lookout for and definitely pick them up if you see them
777 - YES TAKE IT! Then gamble until the gambling machine is broken (you'll probably win way more than you lose), and then if you got a bunch of hearts, throw them into the heart machine to get some really great permanent upgrade boost, at the expense of health.
Rat thing -- This guy homes in on enemies and then bites them to attack for you. Very useful. But the blue cross seen in some levels will attract and freeze him out of commission, so you shouldn't rely on him 100%
Black bullet-looking thing -- This makes your bullets do screen wraparound if there is no boundary on the left/right of the screen (so not valid in all stages). Very very useful toward the end of the game, if you can get this one
red cross necklace and spiked bracelet -- These improve your attack strength so yes get them.
Weird purple head guy -- he shoots enemies to help you. Unfortunately his bullets may be mistaken for enemy bullets, so this may actually make things harder for you, but he can help out. Decide if his extra bullets will make things too cluttered, as to if you want to take him or not.
Cigarettes -- these have to be charged up by collecting enough blue things from defeating enemies but, once charged, you get temporary invulnerability when you use it, so not bad at all, especially against bosses.
Meat -- I think this gives you health back, randomly, when you kill enemies? Not 100% sure. All I know is I was looking very healthy
