As stated in the comments, visit the achievements screen in-game to get those 4 medals. :)
This was a very cute game.
I was a bit concerned about the very short amount of time you got for each level - surely, 20 seconds couldn't be enough for both Derpy AND three muffins? But I found that as I progressed, I was able to solve almost every level in one try, or sometimes two; I only had a few seconds left in most, but it was enough to work out. So kudos for adjusting the difficulty curve so well!
The only exception to this was the last level: the muffins in that one were very well-hidden. I found one pretty quickly, and the second one after a while, but the third eluded me for what felt like an eternity. Of course, I was all the happier when I finally got it and earned the last Muffin medal. ;)
Finding Lyra in the pictures also made for a welcome addition and added replay value to the game. So, all in all: very well done!
