I couldn't stand the high pitch, turbo charged little kid voice. To many people do that and I find it rather irritating.
The ending joke wasn't very funny either.
I did like the kids face when they said he had a mean face. Reminded me of Ren & Stimpy.

I think your somewhat grotesque aesthetic corresponds accurately to the disturbing elements in the novel. However, you did forget to mention the stick sharpened on both ends that is used (we are led to infer) to anally penetrate the twins, as when the owner of said stick spears the pig: "right up her ass!"
well i had to do this rather quickly so i felt that it would be ok to leave out some of the little things, as long as i got it done, and such, but yeah i probably should had mention the pigs more, oh well!
