A lot of room for improvement
The protagonist doesn't resemble anything I've ever seen, you have to die at least six times before finding out what you have to do and as you said yourself, the collision detection isn't good, too.

There is an instructions button on the start screen which leads you to the instructions, so you know what to do. Maybe an ingame help tab would be an idea too.

for an old game it doesnt make anything wrong, but neither does it bring anything new. maybe the cars shouldnt just de-and respawn after you hit them, they could just change there directions and keep on driving. so you could add a few other cars later in the game to make it harder over time.
with re and despawning it gets confusing in a matter of seconds.

The cars used to have a chance to spawn on top of you (hopefully fixed now). That was a pain.
