It reaaally reminds me of the crazy surgeon in Bioshock at the beginning of the game, i think it could have looked like that when he was starting to lose it.
The drawing itself is remarkable, I specially liked how you handled the lights on the scalpel and the glasses, the neurotic look in the eyes and the sweat on the side that gives him a real nervous/desperate demeanor. The hands and the bloodstains are most impressive too!

okay, he's bloody all over and his scalpel and hands, he could be like "STAY BACK DON'T COME NEAR ME" or "don't move, don't wanna miss and hurt you.."
ORRR, he could be trying to do black magic with his hand and he's sweating because he's trying to think about a spell or it's a hard spell to summon, he could be using scalpel to open the wall up so he can escape,
or he's looking at a chick and he's scared of her because she's trying to rape him or something, that's why he has a (so called) boner and that's why he's also sweating..because she's so hot and he's trying think of plan.
Or he could not be doing anything of the above, he could be a candy doctor, the "blood" is really candy juice, and he's sweating because a mad scientist has got some of his candy and turned it into a fucking big candy monster (yummy..)
He's scared, that's why he's sweating, and he's got scalpel to cut the candy monster up.
Or he could just be a mad doctor trying to kill you with his little shitty scalpel then rape you with his boner.

The shadowing makes it look GREAT, the blood looks awesome, and you did a really good job of his hands and face, i'd say this is a 9/10, only because he probably want's to kill me then rape me...
