It's not meant to be in depth, thats the misleading part which makes the flash quirky.

If this is a reference to that scene in which Pyramid Head appears to be violating two monsters... I think he actually, WAS doing it.
Many of the monsters or parts of SH2 have symbolism, related to characters' past and such. During the long sickness of Mary, James had to repress his sexual instincts given the state of his wife. In many reviews about Silent Hill 2's storyline, it's said that PH was acting as a translator of James' subconscious, of his wishes of self-punishment for his crime, and (less likely) of that lack of sex.
Anyway PH was raping a nurse in this vid, so maybe I'm just wandering too much about this. Score is from good drawing and nice enough gag.
