As for me, I always treat anyone who uses a female avatar / character in games etc. as female and vise versa. Even if I know the person is male if they play a female character in a game I will refer to them as a she. People who can't seem to grasp my quite simplistic way of referring to people by their character to help with roleplay usually end up complaining going "it's a guy you know." to which I would simply have to repeat "female character." for the hundredth time to still not get it into their thick skulls.
