This incredibly creepy. There's are many aspects of this that promote the uncomfortable feeling, the background is a strong contender, it reminds me of silent hill with the fog, it also has a lot of emptiness, there are no buildings or other structures besides the lamp posts, and this sends up a big red flag. The Creature itself is clearly not human, nor is it humanoid ("captain obvious over here") especially with that terrifying grin and the eyes. What I'm trying to say is that this is an overall creepy image.
