I've completed a level without moving forward. I've completed a level while holding reverse. I've completed a level holding reverse without ever holding forward. I have absolutely no idea what else could unlock it, so I'm fine with just counting it as broken.
It's a shame, though, since completing it each way was a very fun challenge for which I'd love to have been rewarded.
