I'm pretty sure it's to do with the way that it scrobbles plays from your iPod. If you listen to the same song twice, then sync your ipod, it'll send the data of the "date last played" from the iPod to the computer, as long with the increase in playcount.
Last.FM will intercept this and will know that you listened to that song twice, and the last time you listened to it was at 03:04am 05/06/07 or whenever... however itunes doesn't keep data of when was the time before last you listened to the song... so doesn't remember that. So Last.FM never finds out.
So when it updates your last.fm profile page, it just leaves all of the plays it found at that last listened to time.
Because it knows no better.
....
Of course this is complete postulation and I have not a shred of proof.
But I guess it's something along those lines.
