This is actually a good point. Any other time musicians have made album covers involving infants or children blatantly naked or in "weird" positions a poopstorm has risen.

The Nevermind cover isn't considered to be a "sexualised" image. It's a baby swimming in the water chasing a dollar bill of some sort, conceived as a sort of joke about The Almighty Dollar. Most people are sensible enough to tell the difference between that and child pornography.
Perhaps people are looking at you funny because of the context in which you are talking about "baby dicks".
