No, that's a bad action and anyone who does it clearly is doing it to antagonize you, on purpose, maybe has something against you, I dont' know precisely. What I know is that I don't love it.

That's the zero-bomber problem, it's been around for years, nothings changes, never gonna change.

It's a bitch when it happens, for the exact reasons you specify "how do you know, how to improve it!?"

Don't worry about the scores, they're pretty meaningless, especially when all it takes is some arse to come along and do that, just try to encourage people to review, hope that they review honestly and don't fret about criticism, try to encourage it.
