Technically, remixes re-use portions of audio from the original songs. Many people however label a cover (which is a remake of another song without taking any direct audio from the original) as a remix.
I believe most people here, when covering, rewrite melodies in software programs known as "Digital audio workstations", DAW for short. Others may actually play it on a live instrument and record it.
Assuming you mean "custom program", the software available for this kind of thing isn't exactly custom, as everyone has very similar copies of the software. Custom software would be where it is tailored especially for someone, or actually built by that someone.
