For me, it's Galsia from Streets of Rage 1 and 2. They may not do much damage alone, but in number they're deadly. These guys are actually one of the worst enemies to encounter in the first game just because of their ludicrous speed on harder difficulties, and they flat out surround you.
And in the second game, while they do a lot less damage per hit, they don't have a knockdown attack. Because of this, you better be alert on later difficulties (especially Mania) as they'll do some serious damage in just a couple of seconds.

Sometimes they are harder than the boss fights.

In the older games, they could often knock you into deadly spikes or bottomless pits. In the Metroidvanias, variations turn you to stone and still knock you into spikes or other hazards. They consistently respawn, as well.

Not really. Zelda 2 is a challenging game, but it's not unfair. Iron Knuckles just require a fast reflexes to block their stabs. It leaves you a small window of opportunity to land a hit, but it'll require some patience.

Yeah, the medusa heads can be a huge pain if you don't have holy water and they're place in a really tight space. Well, except in Castlevania 2. They were a complete joke there.

I grinded for hours on octorocks until all my stats were maxed out, and it still takes about 4 hits to deafeat one blue darknut.
Considering the damage they do, how common they are in palaces, and that they are immune to all other practical attacks other than a frontal sword attack, I would say it was built to be unfair (which makes sense to me, as games in that day were expensive and limited, and sometimes had some unfairness in order to rack up the challenge and make said game last longer).
And that's not even counting the blue ones, which can not only hit you at a distance, but speed up after taking a hit.
Not to mention it only takes two darknuts to basically leave it to luck when trying to get a hit in, as they can cover both their weak spots simultaneously.

Zelda 2 isn't really unfair though. Iron Knuckles (not Darknuts, those are the Zelda 1 variations) just require some patience and good blocking skills. You can also use the Shield spell every time you encounter them, which pretty much makes them push overs at that point. Plus, there's always this incredibly cheap strategy. I personally don't use it, but it makes mince meat out of Iron Knuckles, even if there's more than one on screen. Besides, that rarely happens unless you go to far by pushing an Iron Knuckle you're fighting closer to its spawn point.

You have to fight him hand and foot. And will probably die a few times before you figure it out.
I don't think I've played that game in over a decade.

Yans in Final Fantasy IX, because they keep whaling on you with comet for massive damage.

Cait siths in Final Fantasy all of them. They are capable of petrify and the "blaster" move instant kills anyone it hits. Blaster has a high success rate and nobody has immunity to it. They use blaster every other turn. They have lots of HP and dont give you much EXP. for actually beating them.

Bug pokemon in the Viridian forest are a bitch if you get poisoned, and you only have on pokemon. Other ones are Hammer Bros, that fucker on the cloud and the Slicer from Metropolis zone in sonic 2.

Also, pretty much everything in Dragon's Dogma until reaching certain levels. When you're a low level, a fly on your nose could kill you. But even as you level up, there will still be something somewhere that can murder the hell out of you if you try to take it on too early. You gotta be careful where you go and when you go there. The everfall looks fucking tough. Last I was playing, I was just grinding like crazy in preparation for Grigori. I'm off that game for a bit, though. I heard Dark Arisen takes it to a new level, too.
Pretty much anything in Vagrant Story. New player racks up 100 Risk comboing an enemy, then it turns around and wipes them out in one hit.
There's also the Genprey in Monster Hunter. If they stun you right before a big monster attacks, you're finished.
