In firefox at least, you can right click any level-able icon, then choose "view background image" and it will show you a spreadsheet of all the icons.

Yeah, it's a mystery level. Unused. There have been speculations but no answers to what it's true purpose is. Maybe there's room for expansion...

Good knowledge. I always thought the levels were randomly conceived weapons and items that were then sorted in a logical order of fierceness. How many of the NG level icons are more than just level icons? Is there any list or posts of such trivia?

If you look at the list of icons along with the descriptions of them as given by JohnnyUtah you can get a general idea of where some of them were from:

Level 10 is a box cutter from Breaking Bad.
Level 30 is the Boomstick and Level 48 is the Book of the Dead from the Evil Dead Movies
Level 42 is the Glaive from Krull
Level 43 is Belial from Basketcase
Level 45 is the Phantasm Sphere from Phantasm
Level 46 is a Shark with a freakin' laser beam on its head from the Austin Power films
Level 47 is Alien Hominid's Gun

and I've been told that Level 50 is loosely based on Pandora's Box from the Hellraiser series.

That's what I thought too. :) It's even called a 'mystery box' but then again that could mean anything. As for other icons: the rusty weapons don't look very rusty at all. Also never realized the unicorn skull was just a skull, I always saw it as a skull on a stick.. and I always thought the NG sword was actually the head of a spear! You learn new things everyday...
