The first thing that came to mind was the closer for the album Sagas by Equilibrium, Mana. It's a gigantic 16-minute instrumental piece.
Agony by Fleshgod Apocalypse has a great opener and closer. The opener (Temptation) is heavily orchestral, while the closer (Agony) is a damned good piano piece.
Most of Dream Theater's closers are excellent, other than that nothing comes to mind.

I don't know. I listen to a lot of pop albums so it's difficult to determine.
First one that comes to mind is "Disloyal Order of Water Buffalos" for the album Folie a Deux, but I don't really think I want that to be my answer.

The opener for Gorrilaz "Plastic Beach" album was pretty epic and it's closer was perfect. Other than thal Slayer's "Reign In Blood" had a sweet opener. I feel like saying Darkside of the Moon is cliche but that shit was perf

I wouldn't claim that these are the best, but I've always enjoyed the opener to Tim Hecker's Radio Amor and the closer to Night Lights by Gerry Mulligan.
