I don't think I can re-emphasize the exercise point enough. That's your biggest ally in fighting insomnia and out of whack sleep schedules. I'm not so sure your third step helps much at all, and step five is redundant, but the other two are good.
Also, do you use your bed for things other than sleep? If so, stop. Now. Your body will be accustomed to not going to sleep when you're in your bed, where you use your laptop all the time/watch tv/fap for hours. Use your bed for sleep. Nothing else.
