Scariest game? Eternal Darkness! No kidding, when that scary monster appears when the sanity meter goes down! When I first played that game in 2008, I was just 9 and I literally pooped my pants when I saw that monster. Still, one of the best games I have ever played.

When I was five years old, I played Insaniquarium Deluxe, and I was up to get the final egg. But then, a sinister laughter is heard. "BUAHAHAHAHAHAHAhahahahahah!!" That was an octopus. With an glass-shelled brain. I swear, his laughter scared the shit out of me! But now I'm not afraid. I realized his laughter wasn't so scary after all.
But now for sure, the scariest game I SAW (not played) is something about aliens trying to conquer the world. Some spiders, zombies, etc. There are a lot of situations that may fucking scare you. I watched on Youtube, the player wanders with his mate through a hall, and then, suddenly, a giant screaming spider head appears at the screen. That's because the spider's at your face (!!!) Now tell something 'bout that!

Oh yea i remember i played Insaniquarium Deluxe was when i was seven and i was on i think the last one cool game...
and i'm not sure whats it called deadspace? i don't know what's it called...
What I find scary is atmosphere, limited options, and the unknown. One game that does all 3 of these is a 360 game called Decay.
Silent Hill 2. It is the only game so far that I have played which I actually had to stop playing because I was getting freaked out and disturbed, ending up making me feel incredibly uneasy and eventually forced me to turn it off. The best part is that there wasn't even any scary scene etc. that caused such feelings. It was the atmosphere of the town itself cutting into me. Silent Hill 2 is by far the scariest game for the sole purpose of it not actually having scares in it, but still managing to beat those that rely on them to be scary.
Sure games like Fatal Frame are utterly pant shitting scary, but they need scary scenes to push it there, Silent Hill 2 didn't.

Eternal Darkness is a scary game, but it is also a very funny game. Playing with the sanity meter low is both scary and funny to see what sanity effects you will get next.
More games need to take advantage of this meter

Iron Maidens are worse but luckily there are only three of those and at most if I remember correctly, 10 Regenerators, the breathing and wacky eye balls make them weird. Playing against them on professional is worse since they have 6 plaga instead of 5.
This is a pretty hard question, as I've played my share of really scary games.
Resident Evil 2 was quite scary for its time.
I think the meat of Bungie's work, at least back in the day was pretty scary to me.
Their Marathon series was darn near naturally dark. The S'Pht scared me so bad, when my bro 'n' pop encountered what was initially thought to be a "dress". Their Abuse and Pathways into Darkness (pre-cursor to Marathon) were also quite scary games.
Dark Forces was pretty scary in some cases; I couldn't stand being around the phase 1 Dark Troopers, no matter how bright the area I was in or w/e.
REALLY hated the dark things in Ico. Those things were a nightmare.
As scary as games were during my 90s/00s childhood, I hadn't gotten to an actual screaming point until I played the Uncharted 3 demo. Those sudden disastrous moments had me verbally reacting, something I found quite rare for me.

I really really really don't see why so many people find Regenerators and Iron Maidens scary. When I am willing to not only kill every one I can see, but also willingly go backwards into previous rooms to find them so I can kill them for the 5k they drop, it is at that point I am comfortable with saying a creature isn't scary. Scary creatures make you want to avoid them as much as possible, not search for them.

I find them freaky when you shoot off both of their legs, crawl toward you, and lunge at you. It's also freaky when they start running at you as it's rare for them to do that. Finally, it depends on what play style you're going through. Pistol only with no health upgraded? You better just run past them and hopefully they don't grab you from behind. Have the PRL 412 or chicago typewriter? Why the hell are you scared of this guy?

Narrow escape from him, run into a room, pick a hiding spot at the last second he comes in... he walks near where your hiding, does he know where you are?
Uninvited for NES gets an honorable mention. not really scary but some creepy death scenes.
Doom 3 made me jump the most.
Doom 3 gets my vote by a lonng shot. Carrying around a maglight made me feel so freaking defenseless, pluss fighting off imps with only your muzzleflash as light was pretty sweet.
