I love how most of you guys have said Resident Evil 4. All of the art for that game was awesome.
Already been mentioned but I do like Resident Evil 4's cover quite a bit.
The game itself is also my second favorite game of all time (the first being Final Fantasy VII, which has also been mentioned)

I always liked how that cover has Donkey Kong in a cage, while in the game, he's just tied up with a rope. That always made me wonder when I was a nerdy little kid.
Also, Spiderman on the N64 had an awesome looking box. I always loved the unique view it provided. The game itself wasn't half bad either...
This pic proves that not all cover art is awesome. The game's a classic, but the art is horrible.

It's not really "cover art", but i always took a liking to all the classic GTA concept art. Something about the way it is drawn just speaks to me
