I think I've become desensitized to most of it but Dead Girl disturbed a lot of people.

Gummo was actually pretty disturbing
There's gory movies with shock value like the Human Centipede where you just go "That's gross," but I actually felt like Gummo raped my brain because it was so weird and there must have been more to it but I have no idea what. The entire movie just made me feel nasty.
August Underground trilogy, Guinea Pig films, Cannibal Holocaust uncut, and Eraserhead.
but...Gummo wasn't all that disturbing

but not completely disturbing
although i will agree with Eraserhead...

i found it hilarious and badass, but when i think of a normal person, they would probably find it disturbing
