I think I've become desensitized to most of it but Dead Girl disturbed a lot of people.

Gummo was actually pretty disturbing

There's gory movies with shock value like the Human Centipede where you just go "That's gross," but I actually felt like Gummo raped my brain because it was so weird and there must have been more to it but I have no idea what. The entire movie just made me feel nasty.

I found Antichrist disturbing.

Begotten is definitely up there as well. Eraserhead was more weird than disturbing.
