In the Wii version of Goldeneye (I know I've been bringing this game up a lot lately, but I assure you, it is not at all a bad game, just has some flaws) the last chapter (out of six, each being in a different country) I felt like the devs realized it's the last level, and forgot to make a difficulty curve, so they added explosives everywhere that when shot can kill you instantly, added three times as many enemies, and added a horrendous escort mission.
I also found that the elite four in Pokémon Black/White to be much harder than past installments, and the rest of the game.

Wrong.
WaW on Veteran. I remember starting it thinking it wouldn't be too hard. It wasn't, until I was stuck in a spider hole for around 45 minutes, after being absolutely raped by nades, slowly making progress, to then die before a stingy fucking checkpoint.
Bayonetta had one for me, although it may have just been because I'd stopped playing for a while, came back, and suddenly I could't hit anything. It was the burning / sinking / exploading city level, where it introduces the dog wolf twins, electric and fire.
I loved that game, must play it through again one day.
But other then that, pokemon is the only one I can think of. There's an area in every game where there's no more trainers to grind on, and the wild pokes are to weak to hold up a fight, so you're basicly screwed if you're too weak.

The first Modern Warfare
Most of the campaign was pretty straight forward "Shoot, proceed, shoot, proceed" but some of the missions were real shit suckers. One of my most hated missions was the one where you climb up a hill to the barn in order for the chopper to pick you up, then you are told you have to run back down. Not so bad, but then they decide to screw you over with a damn three minute timer. The enemies won't stop spawning until you pass certain parts, and you'll be lucky if you come across a checkpoint, 'cause if not, you start back from the fucking beginning. Quite frankly I hate it in any game where you are given a certain amount of time to complete a mission, or you are to survive or protect something for a certain amount of time.
Two notable examples come to mind:
The High Dragon in Dragon Age 2. The rest of the game wasn't too hard, then this motherfucker swoops down and spams you with dragonlings whilst pummeling you with cheap-ass fireballs. No matter how many times I tried, I couldn't beat him until I went back to Kirkwall and swapped out my party.
And Sean Darcy from Alpha Protocol. There he is with his regenerating armor and infinite army in his cozy sniper spot lobbing dozens of grenades at you, and there you are with your thumb up your ass. Darcy's a bitch because in order to hurt him even a little bit, you need to clear out his goons (temporarily, I might add), dodge his grenades, AND clobber him with enough ammo to break through his armor, only for your magazine to run dry and force you to repeat the process all over again. Oh yeah, and you only get one life... and he hardly ever peaks his head or becomes vulnerable. The only way I beat him was by hiding behind a rock that I could conveniently shoot past, but he couldn't lob grenades over.

WAW on veteran co-op is RIDICULOUS especially on that one level where smoke is every fuckin where and the enemies can still shoot you with pin point accuracy but you can't see shit. Also the AI get's like infinite respwan until you attempt to move up only to die.

Heat.
Absolutely, positively, the hardest mission in that entire game if it glitches on you. It glitched for me so I spawned right between the barn at the top of the hill, and the fields. That meant enemies could see me from the moment I began. Which meant I'd be taking damage from the moment I began. I actually had less trouble with Mile High Club (P90 spraying session) and No Fighting in the War Room than I did with Heat. It's the only mission in the game I had to basically complete twice to finish on Veteran, since I had to do the entire thing again.

Quake 3, the final boss. I couldn't even beat him on easiest mode. Ok, I don't play too many ego shooters anyway, but the level just before the final boss was absolutely no problem for me... and then BAM!

I like to thing GTA 4 had a difficulty spike in the later missions. It was were if you didn't bring body armour, you would die (or maybe that's me).
But Street Fighter 4's fight with Seth is always quite a ridiculous difficulty spike even with the difficulty level on very easy he still can win... sometimes...

agreed, both Dead Rising 1 and 2 start relativly easy and then jumps into (for me) very hard within just a few missions, with no real curve.
I can't think of anything specific, except for Sonic Colors, the whole game I'm doing fine, with no real increase of difficulty, and then suddenly...Astroid Coaster...fuck...

This. Fucking this. I still haven't completed it yet, a few months ago I tried it after a year and I was on the mission called Downfall. It was so fucking hard but I managed to complete it. I thought that was the end of the game, but nope. One more mission. Its impossible so I'm not doing it.
Final Fantasy 5. The third world.
The first time I played this game, I didn't understand the finer tactics of it, the use of blue magic, the proper jobs to delve into etc. The result of this lack of knowledge and time spend leveling the correct things up completely destroyed me. I had no hope of destroying the guardian bosses of the legendary spells and weapons etc. I was completely stuck, I tried going to the end of the game and the random battles raped me. In the end I left my save and gave up.
I had to come back to playing the game a few years later, with tactics and plans burned into my mind to actually be able to beat the game. Seriously the third world of FF5 is a relentless difficulty spike that effectivly says "You haven't learned how to play THIS rpg have you?"
