For me, I'd say it's a tie between Donkey Kong 64 and Spyro: Year of the Dragon. Donkey Kong 64 scared the shit out of me as a kid; most of the levels were extremely dark, (I mean tint wise) the music was creepy, and I always found the game over cutscene unsettling. As for Spyro 3, the game itself was a very upbeat little adventure, but I got really nervous the first time I got to Midday Garden after that one girl told you about a trap being set followed by the ominous music.
So yeah. Discuss or something.
I'd also like to apologize in advance for the poor structure of my post. It's late, and I'm half asleep.
This doesn't count but Dead Space 2 made me jump quite a few times, but Dead Space 1 didn't scare me AT ALL, literally.

The Zelda games freaked me out when I was a kid.
When I first played Donkey Kong 64 I was really scared to go in the water around the island, because I was convinced something was gonna swim out and eat me.

There was this game on the PS2 called Ty the Tasmanian Tiger and was i was little there was this ocean level where there would be tons of sharks. This absolutly scared the shit out of me as i hated sharks back then and could never finish that level without my mum or dad to get past it for me. Once i was able to play that level i was still shitting myself and was pretty much doing speed runs of the level.

I was 5 when I played Half Life 1 when it just came out...
Now consciously I wasn't that much (though being creeped up by a zombie was still pretty fucking scary) but when I went to sleep, that's when the nightmares started, I literally sat in my bed looking through my door into the hallway where I was 100% sure I could see a headcrab zombie standing still looking at me.
After that my dad banned Half Life for me for a LONG time.

It says "Get out." in a creepy voice with the 10 seconds timer starting when the voice is heard, and the room darkens and a cross hair inside a sniper scope points at you, with the unsettling chime added each time a second goes down.
That shit was scary.

^ This. To be honest, a lot of the caves used to scare me. I mean, the music dies down, there's more enemies, and it's all around creepier than outside. It scared the crap out of me back in the day.

Freaking Redead in Zelda: Ocarina of Time scared the shit out of me. I even found the Wind Waker ones kind of unsettling later. Also probably 50% of Majoras Mask.
Super Mario 64's piano monster still haunts me in my nightmares.

I used to be scared playing any FPS really. Shit popping out and shooting me in the face. I could only play Doom with IDDQD, and I didn't play through Half-Life until years later.

There was a ghost character in the Harry potter : philosopher's stone game on the PS1 who scared me when I was 6-7. On the contrary I watched my older brother play games like Resident evil and Silent Hill and I didn't find them scary.

Honestly, Ghadius in Klonoa used to scare me.. the mood is just completely creepy in comparison to the rest of the game and his outfit is just plain weird. The Wii version of the boss looks worse.
