The only reason I ever say "you mad?" to people in TF2 is because they are, in fact, mad. They will then start saying they hate that phrase and will continue to shout profanities, thereby concluding he actually is a hater and is simply too mad for life.
Best response is, in the future, to not put yourself in a position to be asked whether or not you're mad!

Depends on who's asking:
If an elitist-type player, respond with "You SEMI-pro?" If becomes defensive, egg on until they rage-quit.
If TK douche, ignore and return the favor and TK the douce and/or lure TKer to enemy base and while he's trying to kill you, he may actually do some good. If friendly fire is not on and douche is being disruptive, push said person into a geometry hole so he is stuck. Make sure he has run out of grenades so he can't suicide and respawn.
If random person, respond with "?" or "idk" and then you'll know if it was a troll or not.
I never talk online and most of the time I'm calm. You can answer the question yourself sometimes, bad day, was caught of guard. If you are raging that your team just sucks or they aren't going to get the objective then you have a pretty good reason why you should be mad. If you don't wanna hear the phrase and you know your going to rage out loud to your team/other team turn off the mic and just let it out that way. After all "you mad" is only going to amuse the guy who said it.

I was playing TF2 the other day and my team was losing so many times in a row so I said 'I wish this server had autobalance' then this person said 'you mad?' so I said 'You'd be a little mad too if your team was losing x times in a row' and he didn't say anything after that
