I think The Matrix Revolutions should of never been made, in my honest opinion, it ruined the first two Matrix films for me.

Sequels tend to mooch off the success of the first films and that is a tedious thing to do.
An example of a sequel gone wrong I remember is: All dogs go to heaven.
The first film was a beautiful one, the second took it and turned it into a generi kid's movie.
Still waiting for Spaceballs 2: The Search for More Money sequel though

Agreed. That film just went on and on and on, without anything being added to the story. In the end, I never even finished the film because I was bored out of my skull with an hour still to go.

I can go along with this. I can't remember giving any other component of the film a truly negative response, despite readying myself for something considerably more dire before hand. Some thought Coppola's performance wasn't too bad, but I was glad to find that I wasn't in the minority regarding her performance.
It's an interesting example as far as 'bad sequels' go too. (Francis Ford) Coppola was still paying off Apocalypse Now and One From The Heart after all, and while he could clearly direct great films, none of them would be the next Godfather in the eyes of the masses... except perhaps an actual sequel. It was still a big hit, and received Oscar nom nods.

Oh, and someone's going to flame me for this probably, but Army of Darkness. The Evil Dead series should have ended with the second one; Army of Darkness is a fairly light hearted comedy compared to the other two. Doesn't reallly keep with the tone imo.
