Update: after talking with my friend who works in the gaming business he said fallout new vegas would be coming out in June 2010.9 more months.

my best guess is america was the only people to make shetler or vaults.

I think that's part of the mystery, you don't really know what happened.
You can assume that the entire world was destroyed based on the fact that coast to coast in the continental United States, nothing was left, and there's no outside humanitarian effort to rebuild society, but judging by what you see in the game, maybe no one wants to touch this shit with a ten foot poll.
Maybe the rest of the world suffered an economic collapse after America was destroyed (this is based on the life and times of the 1950's), or maybe they got locked in a third World War, were too busy to help, and, going back to the economic collapse, completely bankrupted themselves with it.
Or maybe they all were killed when the bombs fell. You don't know why no one has showed up, why society can't even begin to get back on it's feet outside of California, or anything.
That's what makes it fun.

Also, I totally support post apocyliptic pirates. :3

Obsidian is generally a pretty awesome game company, so this will be pretty good.

The rest of the world DID get hit. In Mothership Zeta you see the planet from space. It's all grey and there's a huge glowing crater in the middle of what I assume is China.

check fallout wikia for all info u need and europe was blown up too and so was aisia and everywhere else
