I thought San Andreas got rid of that trend. I recall being able to dive in that game as well.

Accidentally letting go of the super-long ladder in MGS3, right at the top. (Okay, I did it on purpose, to watch him fall.) Other than that, it simply boils down to water and touching enemies for the worst way to die.

In Killing Floor when you are killing that one dude with the machine gun and rocked launched and you're using the L.A.W and you aim it and right when you fire, a teammate moves right in front of you and you kill youself
