Steps to overcome these problems:
-Consult a doctor about the possibility of a mental condition; this may be responsible for any learning or behavior problems. There is no shame in having one, and if you have one that's undiagnosed it will only cause more problems in the future. Knowing is half the battle; if you have a mental disorder, there are systems in place to help you. Take it from someone who knows.
-School: Pay attention in class, make homework your first priority, and above all try to enjoy learning the material. School's a lot easier if you like it.
-Sports: Maybe athletics isn't your thing. Research martial arts and find one that suits your style; for example, karate for grounded grappling and hard strikes, krav maga for rapid-fire moves, tai chi for physical and spiritual health, etc. (In your case I'd actually recommend tai chi, since it helps to clear your mind.) Or if you want to be able to do moves out of the Matrix, learn parkour; no combat involved, and you can impress people with your mad skills.
-Girlfriend: Resolve the behavior problems first. Girls in general will like a kind, well-mannered guy who takes time out of their day to listen to them. You'll be expected to take initiative, so find opportunities to strike up a conversation. Don't use pickup lines, just ask them out; the worst that can happen is she'll say no. If all else fails, you can still be a happy single.
One final note: the fact you're reaching out for help says a lot. There are plenty of people in your position who don't give a damn; kudos to you for making the effort to change your life.
