This appears to simply be the MIDI file "Megavitamin Jazz" by Dr. Fruitcake, but with a soundfont attached.

Excellent remix!!
Loved it! And I'm glad you added a little tune from Super Mario World and Super Mario Bros. This pretty much the best of Dr. Mario remixes I've heard.

I can hear the distortion but otherwise a nice remix(I wish i could do a remix like this one)

Very creatively done
The only reason I hold back a 10 is because the mixing was not 100% flawless in that at parts the piano (or celesta, possibly?) got distorted as well as a few other hits.
But this piece altogether was very creatively executed and the instrumentation is completely flawless.
Great job!
