Woah , this is so good ! Only thing i would suggest would be smaller eyes to fit the scale of the body and to make him look creepier. But , 5/5 man , well deserved.

I decided to go with the big eyes since everything is quite exaggerated in this picture,but now that I think of it,making the eyes smaller does sound quite neat.

So do you draw starting with a very large canvas? Also do you draw the outline thick to begin with or do you manually go over the edges and thicken it up by sections?

Almost every drawing I made has been drawn on an A4 size watercolor paper.As for you second question I would say the latter.I ink everything with almost the same line size and then I just thick out the areas I need to.
