I had this image of a sadistic surgeon coated in blood with a creepy smile and decided to give it a shot. The idea was that he was an ex surgeon but loved the gory essence to the job and so carries it on in his basement where he does freaky operations. Hes gone a little mad and is obsessed with killing.....hence the writing - I tried to throw in a few made up equation scribblings on the wall to show his cunning side to how he might capture his victims - that he is calculating rather than sloppy. :D
Was aiming for the 'creepy' factor from the grin and colour scheme.
I used a reference for the hand pose (myself) cos I find hands bloody hard to draw lol!

Believe it or not the hands are one of the highlights of this image, they look amazing! They're very well drawn and detailed, though the hairline/forehead area could improve, the rest of it is utterly amazing! Keep that up!
